COVID-19 Release

Dear Amber Spa Patron,

As we plan for our reopening, we do so with no higher priority than the well-being and safety of our guests, team members and community. To this end, we ask that you please review important information posted here regarding new policies and procedures in various areas of our operation including things that we are asking you to do in order to help maintain a safe environment.

This being said, we want to point out that no one should ever forget that this is your getaway, and that we are here to serve you. As service professionals, it is our job not only to deliver a safe and responsible environment but to also provide you with a relaxing and enjoyable experience . At times, this can be a difficult balance to maintain, but it is what we do with creativity and diligent effort. Please know that we look forward to welcoming you back to our spa and will be working tirelessly to deliver the best experience you have come to expect from us over the years.

I knowingly and willingly consent to having an service at Amber Spa during the COVIS-19 pandemic.

I understand that COVID-19 has a long incubation period during which carriers of the virus may not show symptoms but may still be highly contagious. It is not currently possible to determine who has the virus and who does not given the current limits in testing.

I understand that while Amber Spa takes every precaution to mitigate the transition of viruses at their facility, due to the hands on nature of procedures, there is always a risk of exposure to the virus by being in the spa and salon.

To reduce the risk of transmission, I confirm I am not present today with any of the following symptoms of COVID-19: Fever, Runny Nose, Shortness of Breathe, Sore Throat, Dry Cough, or Muscle Pain.

I confirm that I have not been exposed to anyone with know COVID-19 in the past month.

I confirm that I do not work in a nursing home with COVID-19 infections.

I understand that air travel significantly increases the risk of contracting and transmitting the COVID-19 virus and that the CDC recommends social distancing for a period of 14 days upon the return from any air travel.

I verify that I have not traveled domestically within the United States by commercial airlines in the last 14 days.

I verify that I have not traveled outside of the United States in the last 14 days to countries that have been affected by COVID-19.
